Pet Import Requirements in Indonesia
Cats and dogs can be imported only from rabies-free country. The list of rabies-free countries doesn't include USA, but includes Hawaii and Canada. This means that import of pets from USA is banned, but some carriers offer service for sending approved pets from the US and Canada to Indonesia. Birds, cats, dogs, and monkeys are prohibited on some territories of Indonesia, such as Bali, Palembang, and Irian Barat.
Exceptions to the regulation for pets from non rabies free country (USA) include pets for "diplomatic purpose" (pets, whose owners are diplomatic passport holders) and for dogs or cats for show or exhibition, as well as pets for security, defense and public (government) purposes.

Vaccinations and certificates: All pets must have an original Rabies Certificate and the certificate must include information on the microchip number, the date of inoculation and the validity of the particular vaccination the pet obtains (some of them are good for 2 years, others are only good for 1 year.  This vaccine must be an inactivated. Pets also must be up to date on all other necessary vaccinations.

Vet Health Certificate (Form 7001) is the standard Health Certificate to be filled out by the owner's USDA accredited Veterinarian. Must be issued within 10 days of the departure.  Health Certificate must indicate that the pet has no symptoms of Aujossky's disease, distemper, rabies, leishmaoiasis, leptospirosis and tuberculosis.

Each pet entering Indonesia must have import permit. An owner may obtain it by having the copy of passport, copy of valid health certificate from the veterinarian, and the copy of pet's vaccination book.
